Your NewYork-Presbyterian School-Based Health Center has some exciting news: We now have an online consent form! This means that parents/guardians can quickly and easily complete and submit the consent form from their computer, smartphone, or tablet. Our paper form will continue to be available and accepted, but we expect that many parents will prefer the convenience of the online form.
